Discussion on Region II fires since 2021 meeting

While an effort was made to discuss all fire events since the last Region II meeting, only the learnings for the Region are recorded below.

Summer - for IRIS 525603, learning was that there was recall on this switch for CAT diesels that should have been addressed

Sequoyah - for IRIS 520726, learning was that a dirty heating element shorted and caught fire. Sites should test heaters regularly (to burn dirt off) or replace older heaters. For IRIS 508878, moving the trash compactor to a less risky area than next to building was an effective corrective action to prevent reoccurance

Oconee - for IRIS 521487, good to report electrical issues that did not result in a fire, it helps FPRA with risk numbers

North Anna - for IRIS 519955, EDG candle flame fire was accepted condition for previous years but now a intolerance for risk is driving a modification to deal with this issue once and for all.

Brunswick - for IRIS 527129, Hot work - Arch gouging caught fire cloth on fire, put out with smothering - Better work control practices are needed in future arch gouging to prevent arch gouging slag build up

Benchmarking

Hatch - provided info on how to deal with different issues. Desire to speed up input. Let's not let it take away from the meeting.

WBN - AFFF Cart Issues. Not having plant summaries in the morning.

SQN - Foam Cart. Bring Enforcer to next meeting. FF Strategy and Tactics.

BFN - Placement of command post in fires - especially transformers. Does not need to be under power lines.

North Anna - Power Line. KPI for work orders. Hatch is replacing diesels, reaching out.

St. Lucie - Career opportunities. The WO KPI to emphasize the importance of work. Power lines.

McGuire - Sprinkler Head testing

Brunswick - PIV Replacement. Contact info from others who have completed this.

Oconee - Nuclear Community online community. Cheater bars.

Catawba - buried piping degradation and recover / repair. Getting the site to understand the risk with WO and importance. Matching risk and dollars.

H.B. Robinson. Nuclear Community. Good tool. Discussion on Foam Carts.

VC Summers - TFPI information. Discussion on talking with management on FP repairs.

Duke - How are the fleets implementing the new INPO Fire Protection Health report template?

Southern Nuclear is implementing it with no exceptions. Seeing 5-6% drop in health scores due to EQ section for equipment failures

Farley - VESDA Obsolete Detection system- 450 days old - Waiting on Mod

Hatch - Broken Diesel Driven Fire pump - 200 days old - Waiting on Mod to replace this engine

Vogtle 1/2 - NSCW tower detection system - 300 days - No Maint. resources, getting contractor to replace

Vogtle 3/4 - Detection / Automatic Suppression not installed yet in Spent Fuel Pull, waiting on completion of new system installation and turnover

Watts Bar - Electric fire pump OOS due pressure control valve - design issue to replace current valve

Sequoia - Multiple NEIL impairments with three NEIL impairments back to 2013 but no licensing required impairments, need funding for NEIL impairment repair

Browns Ferry - Supervision Alarms (Pre-action system) failures - greater than 450 days - funding and management priority

North Anna - Simplex panel detection system card issue - greater than 450 days - needs ED for obsolete part but have positive path forward to closure

Surry - Hose House in PA with supply piping failed due to graphic corrosion - 500 days - Lingering repair and lack of ownership - developed fire protection excellence team and real improvement in old impairments noted since

St. Lucie - Obsolete and degraded FP detection panels and ECCS detection system field devices - open since 2013 due to lack of funding and priority, changed Work Management Process and have funding now and impairments are on track for closure in near future

McGuire - Detection system OOS (NEIL) due to notification system failure - 6 months, lack of funding and plant priority

Brunswick - Drain system failure to handle multiple system actuation in short time period due to drain blockage, 2550 days, complexity and plant prioritization, on track for closure next outage

Oconee - Multiple hydrant issues, oldest is 2000 days, complexity of repair, rework and work prioritization

Harris - No comment

Catawba - Detection failures in containment - 330 days (Outage required) - Outage require for repair

Robinson - No long term impairments (have Fire protection oversight committee, work prioritization and MFIN team repairing with urgency)

Does your site allow operators to use cheater bars (in addition to valve wrenches) when manipulating hose station valves?

Current Truck can carry 500 gallons of foam.

How many site have larger than 100K gallons of water?

11/17 have greater than 100K gallons storage tanks.

Inspection is required.

Brunswick - Did do a full tank inspection with UT. brought in 20" Tank as temporary storage. Had to have adequate storage tank full of water. After completed some sand blasting, had through wall holes. Do not do during winter. Ended up with a 3 week evolution turning into 5 months.

Concern about water under tank and over concrete slab?
Put in a liner was cheaper than paint.

5 year inspection schedule.

Schedule away from winter months as well as outages.

Can operators stand a watch station and carry fire brigade?

Are operators able to stand watch as well as carry fire brigade.

Multiple approaches. Consensus is that operators can carry watch stations that are not Appendix R stations.

Brandon L. suggested that plants be aware when double assigning duties that they may need to demonstrate the ability to extinguish that fire and perform the required appendix R responses.

Fire Pump Variable Speed Controllers

Variable Speed controllers for Diesel Fire Pumps. - Not favorable.

Looking at better alternatives to prevent water hammer events.

Using different set points.

NRC Inspection. Corrective Action using set points for compliance.

PIV starting to leak by, causing issues on ring header.

Hot taps?

Southern - identified vales faulty. working on selecting strategic valves to isolate and replace. Long term action plan.

Work with license group for strategic plan.

Tackle this a little bit at a time. Attempt to develop a plan to isolate sections.

Considered cutting them off completely and removing them. Then replace them in a tap elsewhere in the line.

Full replacement may be needed if multiple failures.

Many sites are experiencing underground leaks throughout region.

Some pitting on external pipes inward. Even with cathodic protection.

Not gray cast but ductile iron.

50 year Sprinkler Head Testing - how select populations?
