&lt;p&gt;Summer - for IRIS&amp;nbsp;525603, learning was that there was recall on this switch for CAT diesels that should have been addressed&lt;/p&gt;
&lt;p&gt;Sequoyah - for IRIS&amp;nbsp;520726, learning was that a dirty heating element shorted and caught fire. Sites should test heaters regularly (to burn dirt off) or replace older heaters. For IRIS&amp;nbsp;508878, moving the trash compactor to a less risky area than next to building was an effective corrective action to prevent reoccurance&lt;/p&gt;
&lt;p&gt;Oconee - for IRIS&amp;nbsp;521487, good to report electrical issues that did not result in a fire, it helps FPRA with risk numbers&lt;/p&gt;
&lt;p&gt;North Anna - for IRIS 519955, EDG candle flame fire was accepted condition for previous years but now a intolerance for risk is driving a modification to deal with this issue once and for all.&lt;/p&gt;
&lt;p&gt;Brunswick - for IRIS&amp;nbsp;&lt;span&gt;527129,&amp;nbsp;Hot work - Arch gouging caught fire cloth on fire, put out with smothering - Better work control practices are needed in future arch gouging to prevent arch gouging slag build up&amp;nbsp;&lt;/span&gt;&lt;/p&gt;

&lt;p&gt;Surry - Hose House in PA with supply piping failed due to graphic corrosion - 500 days - Lingering repair and lack of ownership - developed fire protection excellence team and real improvement in old impairments noted since&lt;/p&gt;
&lt;p&gt;St. Lucie -&amp;nbsp; Obsolete and degraded FP detection panels and ECCS detection system field devices - open since 2013 due to lack of funding and priority, changed Work Management Process and have funding now and impairments are on track for closure in near future&lt;/p&gt;
&lt;p&gt;McGuire - Detection system OOS (NEIL) due to notification system failure - 6 months,&amp;nbsp;&lt;span&gt;lack of funding and plant priority&lt;/span&gt;&lt;/p&gt;
&lt;p&gt;Brunswick -&amp;nbsp; Drain system failure to handle multiple system actuation in short time period due to drain blockage, 2550 days, complexity and plant prioritization, on track for closure next outage&lt;/p&gt;
