&lt;p&gt;Recommended to take pictures of sprinkler systems prior to shipping in case shipping damage occurs&lt;/p&gt;
&lt;p&gt;(2002) NFPA 25 Section 5.3.1.2* A representative sample of sprinklers for testing per&lt;br /&gt;5.3.1.1.1 shall consist of a minimum of not less than 4 sprinklers&lt;br /&gt;or 1 percent of the number of sprinklers per individual&lt;br /&gt;sprinkler sample, whichever is greater.&lt;/p&gt;
&lt;p&gt;Peach Bottom took four sprinkler heads per system regardless of type&lt;/p&gt;
&lt;p&gt;Head selected should not be just the easiest heads to reach as that may not be representative&lt;/p&gt;
&lt;p&gt;Head testing requirements should include increasing test pressure on heads until they actuate. In addition, the failed heads should be shipped back to the site for retention. These actions will assist with evaluation of the failure and future failures by the site.&amp;nbsp;&lt;/p&gt;

&lt;p&gt;Sequoyah - for IRIS&amp;nbsp;520726, learning was that a dirty heating element shorted and caught fire. Sites should test heaters regularly (to burn dirt off) or replace older heaters. For IRIS&amp;nbsp;508878, moving the trash compactor to a less risky area than next to building was an effective corrective action to prevent reoccurance&lt;/p&gt;
&lt;p&gt;Oconee - for IRIS&amp;nbsp;521487, good to report electrical issues that did not result in a fire, it helps FPRA with risk numbers&lt;/p&gt;
&lt;p&gt;North Anna - for IRIS 519955, EDG candle flame fire was accepted condition for previous years but now a intolerance for risk is driving a modification to deal with this issue once and for all.&lt;/p&gt;
&lt;p&gt;Brunswick - for IRIS&amp;nbsp;&lt;span&gt;527129,&amp;nbsp;Hot work - Arch gouging caught fire cloth on fire, put out with smothering - Better work control practices are needed in future arch gouging to prevent arch gouging slag build up&amp;nbsp;&lt;/span&gt;&lt;/p&gt;

&lt;p&gt;Documented heads replaced by drawing changes&lt;/p&gt;
&lt;p&gt;Oconee reported no failures in testing performed so far&lt;/p&gt;
&lt;p&gt;Wording out of NFPA 25 is representative, not 10%. Can use to leverage less heads being tested&lt;/p&gt;
&lt;p&gt;St. Lucie tested recently and on a 8 or so head system, half of the sprinkler heads failed.
