Peach Bottom took four sprinkler heads per system regardless of type Head selected should not be just the easiest heads to reach as that may not be representative Head testing requirements should include increasing test pressure on heads until they actuate. In addition, the failed heads should be shipped back to the site for retention. These actions will assist with evaluation of the failure and future failures by the site. Sites should test heaters regularly (to burn dirt off) or replace older heaters. For IRIS 508878, moving the trash compactor to a less risky area than next to building was an effective corrective action to prevent reoccurance Oconee - for IRIS 521487, good to report electrical issues that did not result in a fire, it helps FPRA with risk numbers North Anna - for IRIS 519955, EDG candle flame fire was accepted condition for previous years but now a intolerance for risk is driving a modification to deal with this issue once and for all.
