Farley - Last FPTI March 2021, minor issues with combustible controls, blocked sprinkler head. Lesson learned is that we have consider the programmatic element picked in consideration which when it is hot work or transient combustible opens up the whole plant to inspection. Resident violation for Halon system discharge where two dampers did not close for enclosure integrity. Dampers were not tracked as impacting halon system functionality in NFPA 805 monitoring and Unavailability Time was not taken.
Hatch - Last FPTI May 2022, results of no findings. NRC focused on mostly safe shutdown manual operator actions and FPRA. Picked safe shutdown remote panel, intake and control building. They picked hot work and DECP screening reviews
Vogtle 1/2 - Last FPTI June 2021, one green NCV for untimely corrective actions due to closing an action to a LTAM that was never completed, minor for legacy wood transient combustible
Vogtle 3/4 - N/A
Watts Bar - Last FPTI Feb 2021, two green NCVs, one for legacy issue with unprotected cable credited in FPRA as credited, other for "heavily loaded" dusty solder type sprinkler head in front of HVAC duct outlet, would have possibly delayed initiation of sprinkler head
Sequoyah - Last FPTI Sept 2020, no violations with green NCV for pre-conditioning switch in safe shutdown action, Grand Gulf Hot Short issue looked at
Browns Ferry - Last FPTI Feb 2022, Violation on incipient fire detection not being tested per requirements, found issues with air sample manifolds.
North Anna - Last FPTI Feb 2021, no violations, one URI (Un-Resolved Item) about circuit single or MSO analysis, Non-TFPI violation about MSOs for diesel bus due to failure to analyze all modes of failure for some power circuits (App R plant). Resident challenge for discharging CO2 into EDG room for 5-6 seconds accidentally and calling EDG functional.
Surry - Last FPTI Sept 2021, one green NCV for Ops Time critical operator action due to procedure being incorrect on breaker number and not being prioritized appropriately for correction.
St. Lucie - Last FPTI Sept 2019, no findings, a lot of conversation about when T-0 is established, annual drop test being required on fire dampers (not required in COR)
McGuire - Last FPTI March 2022, no violations
Brunswick - Last FPTI Feb 2022, no violations
Oconee - Last FPTI Feb 2022, two green violations for safe shutdown (procedural issue and circuit (pump motor control power supply) that would not operate per manual safe shutdown actions)
Harris - Last FPTI Nov 2021, one minor violation for due to untimely PMT maintenance on detection system. Resident violation green NCV for transient combustible without permit as required by procedure (metal cart with 30 gallons of oil in hallway)
Catawba - Last FPTI March 2021, no violations
Robinson - Last FPTI Sept 2020, no violations
VC Summer - Last FPTI Aug 2019, no violations.

Harris - Nov 2021 - 1 minor violation, safe shutdown analysis. Resident Violation. Transient Combustible. Green - NCV. No permit on an item.

North Anna - Feb 2021 - none. Fire PRA, not NFPA 805. Timer in CO2 panel. Cam Timers. Discharged 10 secs of CO2 into Diesel Generator room. Is it possible to put so much CO2 in the room that the diesel wouldn't start?
Oconee - Feb 2022 - Safe Shutdown issues. 2 Green.
Robinson - Sept 2020 - None
Sequoyah - NCV - Green. Implementing verification of remote safe shutdown transfer switches. Comments on Ground Fault Equivalent Hot Shorts.
St. Lucie - July 2019. None. When Time zero was established for SSD. Recommended annual tests on fire dampers.
Surry - Sept 2021. 1 - NCV-Green. Operations SSD Procedure. Time critical action - wrong breaker number.
VC Summer - August 2019. No Violations.
WBN - Feb 2021. 2 NCV-Green. Latent design error. CCS. FCR for 6 foot clearance for protection. And 2) Dusty Sprinkler head.
